Our new patient forms can be downloaded and completed in the convenience of your own home.
You will be warmly welcomed on your initial visit to the Center for Health. New patients often remark how “modern” our office is, which emphasizes our goal of offering state-of-the-art chiropractic care in a comfortable surrounding. You’ll find that parking is convenient as it is complementary and it is ideally located beside the building.
Make Yourself at Home
You’ll be greeted by name when you enter our office and we will give you a short tour to acquaint you with our facilities. This visit consists of a consultation and an examination/assessment. The purpose of your consultation with the doctor is to discuss your health goals and to determine if chiropractic is likely to help you. If it looks promising, a thorough examination will help identify the cause of your problem. -
